Michelle Howard
Director - Healthy Communities
I have 20 years of experience working in social policy and health roles and have worked across various health organisations, from roles in Acute Care, to Primary Care Trusts, and Clinical Commissioning Groups. I have also worked in Public Health Programme Management and Commissioning roles both in the NHS and local government. I'm passionate about community and patient engagement in service design and development.
Denise Gabriel
Social Prescribing Manager
I’ve worked with some great teams in Birmingham City Council, Primary Care, Clinical Commissioning Groups and Regeneration projects to develop services to improve the health and wellbeing of the communities we live in. My passion is to make a difference and to be part of the change. My inspiration is my Mum; she worked as an NHS Mental Health Nurse.
Leila Yafai
OHP - Bordesley East PCN
I cover 7 surgeries, bases in the Bordesley East PCN area. I support patients by improving their health and wellbeing. My background is in Education, Health and Social care, working with young people, families, and communities. I am most passionate about supporting people in the community.
Naeed Akhtar
OHP - Northfield and Bournville PCN
I have worked in an early years setting for almost 10 years. I have a foundation Degree in Playwork, and have a BA Honours in Professional Studies in Community Family and Youth. My previous role consisted of supporting families and children in health promotion and physical exercise.
Camillia Rhoden
OHP - Alliance of Sutton Practices PCN
I’ve worked in mental health and social prescribing for over 3 years – I have experience with setting up groups with young people who have anxiety and depression in a counselling setting. I am passionate about supporting patients and encouraging behaviour change, I also enjoy the flexibility and versatility of  each GP practice I work with. 
Jackie Moore
OHP - Rubery, Bartley and Weoley PCN
My background is in health care delivery and management and experience in supporting homeless people with access to housing support, employment and training. I have also got experience in delivering sexual health services, volunteer recruitment, community engagement and both setting up and facilitating group support sessions.
Rita Kambo
Project Support - Insight & Holistic Interventions
I provide project administration for our Data and Insight team and Holistic Interventions which involves high quality administration skills for GP/Holistic referrals, managing the Holistic interventions emails box and supporting focus groups.
Keiran Mckenzie
Head of Social Prescribing and Lifestyle Interventions
I have more than ten years experience working in primary care health promotion, public health research, and community service management. I have strong connections to some of Birmingham’s health and community leaders and love to get involved in supporting grassroots initiatives that build community health. I enjoy travelling, meeting new people, learning new things, and writing music.
Josephine Conneh
Sutton Coldfield Group Practice PCN
I have worked in Public Health and Nutrition supporting overall health and wellbeing. I have worked with clinical and none clinical teams to deliver diabetes interventions and also supporting people with weight loss goals. I enjoy attending professional events in the Health sector, new people and keeping updates on industry leads.
Grace Myers
OHP - Moseley, Billesley and Yardley Wood PCN
I have a degree in International Development and have experience volunteering for development NGOs in Nigeria and Peru. I worked in fundraising for McMillian, Cancer Research and Merlin. I have taught gardening, art and hula hoop classes for people of a range of abilities, ages and backgrounds.

I am a qualified & experienced Personal Trainer & Fitness Instructor including Nutritional Weight Management. I have many years of eperience in delivering a variety of fitness sessions with local communities. I enjoy helping and supporting people to feel better about themselves.

OHP - Kingstanding, Erdington & Nechells PCN
I am a qualified social worker with 15 years of statutory experience working with adults. I use a person centred approach ensuring individual can make informed choices about their own wellbeing. I have extensive experience in connecting people with community resources helping achieve their desired outcomes. I am passionate about supporting people to empower themselves
Mundev Kaur
OHP - Quinton and Harborne PCN
My professional background is in community and mental health specialising in supporting people with anxiety and low mood. I enjoy working in the community sector and I am passionate about working with people. In the past I have also worked in the area of conservation promoting the beneficial impact that being outdoors has on people's wellbeing.
Eliza Choudhury
Project Support Officer
Most of my working life, I have worked in Primary Care – General Practice, supporting various roles. Working within General Practice, I have seen the value of social care and how taking a holistic approach is just as important as conventional medicine. I love working with people from different cultures, religions, and backgrounds. I am very much a people-person and love to help
