Hello and welcome to the The Active Wellbeing Society (TAWS) Social Prescribing Service for Community Hall Green Health PCN!

Our social prescribing team would like to invite you to our upcoming support groups with Birmingham Adult Education/The Active Wellbeing Society and other amazing organisations. This is a great opportunity to meet people locally, share your experience of managing your diabetes and learn from others what has helped them in their journey. It is also a great opportunity to learn about the support and resources you can access in the community. The groups are free of charge to attend. The duration of these sessions are monthly: For 1 year (EST June until March 2024).
