Denise Gabriel
I’ve worked with some great teams in Birmingham City Council, Primary Care, Clinical Commissioning Groups and Regeneration projects to develop services to improve the health and wellbeing of the communities we live in. My passion is to make a difference and to be part of the change. My inspiration is my Mum; she worked as an NHS Mental Health Nurse.
Leila Yafai
I cover 7 surgeries, bases in the Bordesley East PCN area. I support patients by improving their health and wellbeing. My background is in Education, Health and Social care, working with young people, families, and communities. I am most passionate about supporting people in the community.
Naeed Akhtar
Birmingham Alliance
I have worked in an early years setting for almost 10 years. I have a foundation Degree in Playwork, and have a BA Honours in Professional Studies in Community Family and Youth. My previous role consisted of supporting families and children in health promotion and physical exercise.
Rita Kambo
I provide project administration for our Data and Insight team and Holistic Interventions which involves high quality administration skills for GP/Holistic referrals, managing the Holistic interventions emails box and supporting focus groups.
Shenique Green
My overall experiences have been in the field of support, having previously worked as a family support worker with children, young people, and adults who have been affected by illness or poor mental health. When I support others, I strive to bring a positive experience and take a non-judgmental, positive attitude. Which will assist me in achieving my long-term goal of becoming a qualified counsellor.
Anna Walker
Hi my name is Anna Walker, I am a social prescriber working in Quinton and Harborne PCN. I have extensive experience working with a diverse range of people from different social and cultural background. I have worked in substance misuse and mental health field. I am training as a counsellor, the social prescribing role enables me to use skills the that are necessary for my training.
Josephine Conneh
I have worked in Public Health and Nutrition supporting overall health and wellbeing. I have worked with clinical and none clinical teams to deliver diabetes interventions and also supporting people with weight loss goals. I enjoy attending professional events in the Health sector, new people and keeping updates on industry leads.
Lauren Dooley
Hi my name is Lauren Dooley, I believe that Social Prescribing is about empowering you to take the steps to improve your health & wellbeing. We help you to evaluate where you are at, what matters to you and set goals for the future; linking you into service and activities that will help you on your journey towards wellbeing.
Damien Phillips
My name is Damien Phillips; I am part of the social prescribing Team for Bordesley East PCN. We cover surgeries such as The Swan Medical, Garrets Green Lane Surgery, Poolway Medical, Sheldon Medical Centre and The Iridium Medical Practice. I am very passionate about helping people, and linking them into the best services, community activities across Birmingham to aid their physical and mental well-being.
Sharon Mullard
Hi, I’m Sharon Mullard and I work as a Social Prescribing Link Worker in the East of Birmingham for patients that have been referred by their GP’s and I help to connect people to activities that are happening in their local communities it is a fantastic way of making new friends and one of the easiest ways to make a positive change to your life.
Mundev Kaur
My professional background is in community and mental health specialising in supporting people with anxiety and low mood. I enjoy working in the community sector and I am passionate about working with people. In the past I have also worked in the area of conservation promoting the beneficial impact that being outdoors has on people's wellbeing.
Kath Richards
Hello my name is Kath Richards and I have worked as a Social Prescriber for over 2 years. Its great to see how the role is growing and flourishing and GP Surgeries are now utilizing the Social Prescribing service. As a Social Prescriber I will focus on what matters to the individual and connect them to local community services and activities.
