Community Health Hall Green – Ackers Adventure Activities

These sessions will be delivered at The Ackers Adventure Centre. This programme is for individuals within the community who have learning disabilities and additional needs. Please carefully select the relevant age group and GP practice to you. You'll need to specify what type of learning disability or/and additional needs you have for this registration to be submitted successfully.

The Active Wellbeing Society works closely with organisations in the community to reach individuals far and wide. You will have been sent the link to this webform by your GP Practice. You will be contacted in due course by somebody with the full details of these sessions.

Only those part of these two age ranges will be able to participate in these particular activities.

The Wellbeing Card is required to enable you to access the free activities through The Active Wellbeing Society. You will be issued with a membership card known as Wellbeing Card which you will receive in the post.
